Fatality #1 for Coal Mining 2016

c01On Monday, January 4, 2016, a 53 year-old belt foreman/fireboss with 34 years of mining experience was fatally injured when he came in contact with a moving underground belt conveyor. The victim was preparing to change out a hold up roller when he was caught by the moving belt and pulled into the roller.

Best Practices

  • Never perform work on a moving conveyor belt.
  • Ensure that power is off with a visual disconnect before any work is performed, use your own lock and tag.
  • Ensure that machinery is blocked against motion before performing maintenance or repairs.
  • Always identify safety hazards before beginning any task.
  • Stay out of areas along a moving conveyor belt where clearance is restricted.
  • Cross moving conveyor belts only where suitable crossing facilities are provided. (e.g. cross-overs or cross-unders).
  • Ensure all guards are adequate and securely in place where exposed moving machine parts may be contacted by persons.

Fatality #1 for Metal/Nonmetal Mining 2016

On February 26, 2016, a truck driver delivering multiple sections of polyurethane pipe was struck by a section of pipe during the unloading process. A forklift removed two sections of pipe from the passenger side of the truck, and then left the area with the two sections. While the forklift was away, a single, unsecured section of pipe rolled off on the driver’s side of the truck and struck the victim. Each section of pipe was approximately 50’ long and weighed approximately 1,750 pounds. Miners began first aid but the driver was unresponsive. He was transported to the local hospital and later died.

Best Practices

  • Analyze all tasks, identify possible hazards prior and eliminate the risks prior to beginning work.
  • Before beginning work, establish safe work procedures, train and discuss with all persons performing the task.
  • Examine work areas during the shift for hazards that may be created as a result of the work being performed.
  • Evaluate the stability of the material before unfastening a load. Pay particular attention to loads that may have shifted or become unstable during transport.
  • Install secondary supports (side stakes) of adequate height and strength to prevent material from falling when the load is unfastened; or prior to unfastening the material, secure the item being unloaded to the machine used in the unloading process.
  • Stand clear of items of massive weights having the potential of becoming off-balanced while being loaded or unloaded.
  • Implement measures to ensure persons are properly positioned and protected from hazards while performing a task. Ground personnel should be highly visible. Unauthorized persons should be kept clear of the area.

New MSHA Web Page is Nice, but…

MSHA has a wonderful new arrangement to it’s web page. Truly, it seems easier to navigate and more logically arranged than ever, BUT it also means that many of the hundreds of links from this site may no longer work. You’ll know because instead of what you’re looking for you’ll find the dreaded 404 Error page. If you’re in a hurry you can probably find the page you’re looking for by replacing the “www” in the address on the page where you found the 404 Error message with “arlweb”.

Fatality #17 for Metal/Nonmetal Mining 2015

m17On December 28, 2015, a 42-year old miner with 3 years of experience was killed at a surface gold mine. The operator of a loaded haul truck was attempting to have his truck climb a snow covered access road when his truck slid backwards striking the cab of the victim’s loaded haul truck, which was also recovering from sliding backwards down the same access road. Several minutes later, a third loaded haul truck also slid down backwards while attempting to climb the access road, colliding with the other wrecked haul trucks.

Best Practices

  • Maintain control of equipment at all times, making allowances for prevailing conditions (low visibility, inclement weather, etc).
  • Haulage roads should be examined for hazardous conditions prior to permitting equipment access and especially when conditions change due to snow, ice, or water. Communicate hazardous conditions to other persons using the haulage road.
  • Keep roadways clear and safe for travel. Remove snow and ice which may cause loss of traction for equipment along roadways.
  • Train all employees on proper work procedures, hazard recognition and avoidance.
  • Observe all speed limits, traffic rules, and ensure that grades on haulage roads are appropriate for haulage equipment being used.
  • Maintain appropriate distance between vehicles to allow for corrective action.
  • On snow covered steep grades, consider the use of chains for better traction while stopping or climbing.

Fatality #16 for Metal/Nonmetal Mining 2015

m16On December 15, 2015, a 75-year old tow truck laborer was killed at a cement plant.  As the tow truck operator was lowering the truck’s boom it struck the victim. The victim suffered a severe head wound but was conscious when transported to a local hospital but later died of his injuries.

Best Practices

  • Position yourself only in areas where you will not be exposed to hazards resulting from a sudden release of energy.  Be aware of your location in relation to machine parts that can move.
  • Establish communications between equipment operators and machine helpers.  Make sure those around you know your intentions.
  • Positively block machine parts (including hydraulic boom lifts) and suspended loads from motion prior to entering areas underneath them.
  • Ensure that persons are properly task trained regarding safe operating procedures before allowing them to operate mobile equipment and monitor work to ensure procedures are followed.
  • Ensure that all operating systems and safety features on mobile equipment are maintained and functional at all times.
  • Operate all machinery in accordance with manufacturers operating guidelines.
  • Wear all appropriate personal protective equipment.

20th Professional Development Mine Safety Seminar for Supervisors

We’d like to be the first to wish you holiday greetings, but of course we’d have to have done that in August or something like that. 😉 Seriously, we know we’re not the first, but don’t want to be the last either, so be safe. No time is more precious than today whether it’s before a holiday or not. We want to remind you of that here at Complete Safety Solutions. Give the gift of yourself and be sure to do your work the way you know it’s supposed to be done, safely. In spite of a bad start, it looks like mining may end up with at least a better year than last year when it comes to fatalities. Still, too many homes are missing a loved-one this season that they shouldn’t have had to miss. It’s hard to say how we can prevent all mining related deaths, but it’s very simple to prevent the next one. Do your share and work safely.

October is the “Deadliest Month”

Historically, October has been the most deadly month for the metal and nonmetal mining industry, with 51 fatalities occurring in the month of October since 2000. During this fall period MSHA intends to continue enhanced enforcement, education and outreach with a special emphasis on the historically high number of mining deaths.

 

Particular attention will be paid to intermittent mines that are beginning to shut down and prepare for the winter season, and the many full time operations that are performing annual repairs in advance of inclement weather. During these activities, many miners may be performing new or unfamiliar tasks, or may be assisting maintenance personnel on jobs they rarely perform, using unfamiliar tools and equipment. Mine operators must assure miners are adequately trained to do those tasks and are protected from hazards.

 

MSHA’s increased vigilance during this period of historically higher deaths will focus additional resources on increased enforcement attention, education and outreach, including walk-and-talks. We are encouraging the mining industry to do the same. MSHA has provided the mining industry with information on mining deaths and best practices to prevent them, particularly since the fall of 2013 when mining deaths increased. Please make sure that information gets distributed. MSHA will be looking hard for the conditions identified leading to those deaths. MNM inspectors, joined by Coal inspectors and Educational Field and Small Mines Services personnel will be visiting mines, calling attention to potentially hazardous tasks and conditions and discussing safe work practices with miners. Please join MSHA in this special safety outreach and help us spread the word to the miners at your operations. This is the time to be proactive. Assure that workplace examinations are being conducted to identify and fix hazards, and that miners are properly trained, including task training to recognize and avoid hazards so they can go home safe and healthy at the end of each shift.

Fatality #15 for Metal/Nonmetal Mining 2015

m15On August 3, 2015, an 18-year old truck driver (seasonal associate) with 9 weeks experience was killed at a granite mine. The victim backed his truck under a conveyor belt to be loaded. After exiting the truck, the victim entered a door leading underneath the “sand fines silo.” Soon after entering the silo, the structure collapsed burying the victim beneath the falling material.

Best Practices

  • Routinely examine metal structures for indications of weakened structural soundness (corrosion, fatigue cracks, bent/buckling beams, braces or columns, loose/missing connectors, broken welds, spills of stored solids, etc.).
  • Periodic detailed inspections should be performed which examine hopper and wall thicknesses, critical connections such as the hopper to the wall, and the material flow conditions. Both the inside and outside of the structure should be evaluated.
  • Report any changes in the discharge flow pattern which may be a result of an internal obstruction that causes non-uniform pressures on the silo structure.
  • Report all areas where indications of structural weakness are found.
  • Schedule inspections of the silo’s interior surface only when all material has been removed to determine if it has become polished and worn from use.

Fatality #14 for Metal/Nonmetal Mining 2015

m14On August 3, 2015, a 26-year old miner with 4 years of experience was killed at an underground gold mine. The drill was traveling in the reverse direction of travel up a 10% slope and was carrying a 13½ ft. long drill steel in a rack that had been installed on the machine. The forward end of the drill steel struck a rib causing it to be pushed back toward the operator. The drill steel struck and killed the operator, and caused him to fall to the ground. No witnesses were present at the time of the accident.

Best Practices

  • When mobile equipment is equipped with seat belts they should be worn at all times when operating that equipment.
  • Loads on mobile equipment shall be properly secured and positioned safely prior to moving equipment.
  • Miners should operate mobile equipment at speeds consistent with the type of equipment, roadway conditions, grades, clearances, visibility, and other traffic that allow them to maintain control at all times. Maintain all roadways free of materials that may pose a hazard to equipment operators. This includes materials on the floor and protruding from the ribs, back, or walls.
  • Keep mobile equipment operator’s stations free of materials that can impair the safe operation of the equipment. Ensure that equipment controls are maintained and function as designed.
