MNM Fatal – 3/28/26

On March 28, 2026, a contractor died after falling approximately 37 feet through an unsecured roof panel of a shaker screen deck while conducting roof repairs.

Best Practices

  • Provide and maintain safe access to all work areas.
  • Always use approved fall protection systems when working at heights where a fall hazard is present. Ensure fall protection has a suitable fall arrest connection and secure anchorage system.
  • Ensure miners are properly trained for the assigned task, including the proper use of Personal Protective Equipment (PPE) and recognizing fall hazards. 
  • Conduct a thorough inspection of the workplace and all required PPE before commencing any work activities to identify and mitigate potential hazards.
  • Ensure contractors receive appropriate training prior to performing work.

MNM Fatal – 10/16/25

On October 16, 2025, a miner was performing work from an elevated platform when they fell approximately 30 feet. The miner died from his injuries.

Best Practices

  • Provide safe access to all work areas by installing handrails, guarding, or the use of fall protection equipment where there is a risk of miners falling.
  • Provide and ensure proper use of fall protection when a fall hazard exists.  Ensure fall protection has a suitable fall arrest and secure anchorage system.
  • Identify risks and eliminate or control them before beginning activities.
  • Conduct workplace examinations to identify conditions that may adversely affect the safety or health of miners.

Safety Alert: Violations, Injuries & Fatalities

As of April 1, 2025, 2,201 injuries were reported to MSHA since October 1, 2024. Fourteen were fatal injuries. For the remaining injuries, 1,523 were non-fatal and resulted in days lost or restricted duty (NFDL), 27 resulted in a permanent disability, and 637 resulted in no days lost or restricted duty. – MSHA

MNM Fatality – 9/20/24

On September 20, 2024, a miner died after he fell approximately six feet from a front-end loader while attempting to replace a bulb on the right front headlight. 

Best Practices

  • Develop and implement a Safety Program for Surface Mobile Equipment that includes actions taken to identify hazards and risks to reduce accidents, injuries, and fatalities related to surface mobile equipment.
    • Identify hazards and risks.
    • Take corrective actions to eliminate / reduce risks.
  • Provide and maintain safe access to all workplaces and establish safe work procedures.

MNM Fatallity – 8/22/24 OK

On August 22, 2024, a contractor was installing a material feeder when they fell approximately 21 feet through an opening in the walkway platform.  The contractor died from his injuries on September 6, 2024.

Best Practices

  • Conduct workplace examinations to identify conditions that may adversely affect the safety or health of miners.
  • Provide and maintain safe access to all working places.  Use personnel lifts or ladders to access elevated workplaces safely.
  • Provide and ensure proper use of fall protection where there is a danger of falling.  Ensure fall protection has a suitable fall arrest connection and secure anchorage system.
  • Identify risks and eliminate or control hazards before beginning maintenance activities.
  • Train miners to perform tasks safely and use tools and personal protective equipment properly.

MNM Fatality – 8/4/24

On August 4, 2024, a contractor suffered severe chemical and thermal burns after he stepped into a deep opening in the floor obscured by puddled liquid. The contractor fell into a mixture of hot water and caustic liquid. On September 2, 2024, he died from his injuries.

Best Practices

• Install adequate warning signals and barricades for openings above, below, or near travelways where miners, including contractors, or materials may fall.
• Conduct workplace examinations to identify conditions that may adversely affect the safety or health of miners.
• Keep workplaces, passageways, storerooms, and service rooms clean and orderly.
• When wet processes are used, maintain drainage and provide false floors, platforms, mats, or other dry standing areas where practicable.

Coal Fatality – 8/4/21

This fatality’s Final Report was first posted on MSHA’s website on 11/7/23. No alerts or preliminary reports were produced.

On August 4, 2021, at 8:12 a.m., Jeffery Hudnall, a 60 year-old steam truck operator with 16 years of mining experience, was fatally injured while steam cleaning a front-end loader.  Hudnall was standing on the front-end loader’s right-side deck when he fell over nine feet to a concrete pad.

MNM Fatality – 8/5/23

On August 5, 2023, a customer truck driver fell from a large container mounted on the trailer of his truck. After opening the container lid, the driver slipped while descending the container.  The driver died from his injuries on August 7, 2023.

Best Practices

  • Provide fall protection or safe means of access to miners, contractors, and customers who need to gain access to containers or tankers if there is danger of falling.
  • Ensure miners, contractors, and customers wear proper slip resistant footwear and use three points of contact when accessing elevated surfaces.
  • Provide site-specific hazard awareness training to customers, including commercial over-the-road truck drivers.

MNM Fatality – 6/8/23

On June 8, 2023, a miner died after climbing over the handrail onto a conveyor belt to gain access to a magnet belt that needed adjustment. When the miner stepped onto the magnet belt, the belt started, throwing the miner 16 feet to the ground below.

Best Practices

• De-energize, lock-out, tag-out, and block machinery against motion before performing repairs or maintenance on a belt conveyor.
• Install a system which provides visible or audible warning to warn miners that the conveyor will be started.
• Provide and maintain safe access to all workplaces and establish safe work procedures.
• Use fall protection when a fall hazard exists. Ensure fall protection has a suitable fall arrest and secure anchorage system, and that miners are properly trained.
