MNM Fatal – 12/22/25

On December 22, 2025, a contractor died after receiving burns from a pressurized hydraulic line that ruptured and caught fire.  The contractor was cutting shims from a cone crusher with a torch.

Best Practices

  • Never apply heat to or near pressurized hydraulic lines.
  • Never apply heat to components when flammable, combustible, or explosive materials are present.
  • Protect and/or relocate hydraulic lines to prevent physical damage.
  • Examine all components and surrounding areas before applying heat, cutting or welding. Conduct daily inspections of hydraulic equipment, hoses, and connections for leaks, wear, or damage.
  • Always use proper protective equipment for the task.

MNM Fatal – 12/10/25

On December 10, 2025, two contractors died when the dragline they were operating slid into a water-filled pit.

Best Practices

  • Establish mining methods based on geological conditions such as soil stability subsurface cavities.
  • Evaluate all pit, highwall, slope, and bank conditions at least daily or more often, if necessary, to protect the health and safety of the miners. Be especially vigilant after each rain, freeze, or thaw.
  • Conduct effective workplace examinations in areas where miners are working and traveling. Identify and correct all hazards.
  • Train miners on equipment risks and to recognize potential hazards that can decrease bank and slope stability.

MNM Fatality – 9/13/25

On September 13, 2025, an electrician contacted 277 volts of alternating current while working on a light fixture in an office building. He was electrocuted and fell off a ladder.

Best Practices

  • Always lock out, tag out, and test electrical equipment to verify all power has been de-energized prior to working on it.
  • Label power switches to show which units they control, unless identification can be made readily by location.
  • Leave protective covers in place until electrically powered equipment is de-energized.
  • Establish safe procedures before beginning work and discuss them with all miners involved in the task.
  • Always use properly rated tools and personal protective equipment when working with electricity.
  • Ensure that miners are properly task-trained for their assigned tasks.

MNM Fatality – 10/28/25

On October 28, 2025, a miner died after the haul truck he was driving went over a berm along the haul road and into a water-filled pit.

Best Practices

  • Always operate mobile equipment at safe speeds. Consider the roadways, tracks, grades, clearances, visibility, traffic, and type of equipment used.
  • Ensure berms and guardrails are at least mid-axle height of the largest mobile equipment using the roadway.
  • Follow the Surface Mobile Equipment Safety Program at your mine.
  • Always wear a seatbelt while operating mobile equipment. Never attempt to exit or jump from an out-of-control vehicle.
  • Perform pre-operational examinations of mobile equipment.

MNM Fatality – 8/25/25

On August 25, 2025, a miner died after the haul truck he was driving went over a berm along the haul road, over a highwall, and overturned into water in the pit.

Best Practices

  • Always operate mobile equipment at safe speeds. Consider the roadways, tracks, grades, clearances, visibility, traffic, and type of equipment being used.
  • Follow the Surface Mobile Equipment safety program at your mine.
  • Clearly mark hazards along roadways with signs and signals.
  • Always wear a seatbelt while operating mobile equipment. Never attempt to exit or jump from an out-of-control vehicle.
  • Maintain control of mobile equipment while it is descending, especially around tight turns.
  • Ensure miners are adequately task trained in the safe operation of all equipment they operate.
  • Perform pre-operational examinations of mobile equipment.

MNM Fatality – 3/5/25 IL

On March 5, 2025, a miner was fatally injured at a surface mine when flyrock from blasting operations struck him.  The miner was assisting in detonating the explosives. 

Best Practices

  • Remove all persons from the blast area unless using suitable blasting shelters to protect persons from flyrock.  Wait at least 15 seconds after the blast for any flyrock to drop and settle before exiting the shelter.
  • Adjust stemming depth and/or decking to maintain adequate burden on all sections of the blast hole.  Consider geology, face geometry, and surface topography when developing a drill pattern.
  • Determine the actual burden for all face holes along their length and adjust the explosive power factor along the borehole accordingly.
  • Only use approved capacitor discharge or generator blasting machines.

MNM Fatality – 8/22/24 MA

On August 22, 2024, a miner operating an excavator died when the highwall he was working near collapsed on the excavator.

Best Practices

• Use mining methods that ensure highwall stability and safe working conditions.
• Examine and, where applicable, test ground conditions in areas where work is to be performed, prior to work commencing, after blasting and as ground conditions warrant during the work shift.
• Scale highwalls to eliminate hazards, e.g., steep slopes or overhangs.
• Ensure that miners work, travel, and operate mining equipment at safe distances from highwalls.
• Train miners to recognize hazardous highwall conditions.

MNM Fatallity – 8/22/24 OK

On August 22, 2024, a contractor was installing a material feeder when they fell approximately 21 feet through an opening in the walkway platform.  The contractor died from his injuries on September 6, 2024.

Best Practices

  • Conduct workplace examinations to identify conditions that may adversely affect the safety or health of miners.
  • Provide and maintain safe access to all working places.  Use personnel lifts or ladders to access elevated workplaces safely.
  • Provide and ensure proper use of fall protection where there is a danger of falling.  Ensure fall protection has a suitable fall arrest connection and secure anchorage system.
  • Identify risks and eliminate or control hazards before beginning maintenance activities.
  • Train miners to perform tasks safely and use tools and personal protective equipment properly.

MNM Fatality – 5/8/24

On May 8, 2024, a miner was electrocuted while unloading a roll of belt from a trailer.  The miner was performing rigging duties when the crane boom contacted an overhead high-voltage powerline.

Best Practices

  • Ensure that booms or masts of equipment are not operated within 10 feet of any energized overhead powerline.
  • Designate one person on the ground, located in a safe location, to give signals to the crane operator and watch placement and proximity of the boom.
  • Use non-conducting tag lines to steady loads when operating cranes where overhead power lines are present.
  • Post warning signs at all approaches to overhead powerlines; install barricades where necessary.

MNM Fatality – 7/26/23

On July 26, 2023, a miner suffered serious injuries when he lost control of the pickup truck as it ascended a haul road. The victim died several months later from his injuries.

Best Practices

  • Perform pre-operational examinations of mobile equipment.
  • Maintain control of self-propelled mobile equipment while it is in motion.
  • After repair and maintenance, ensure mobile equipment is safe to operate.
  • Always wear a seatbelt and remain in the cab with your seatbelt on while operating mobile equipment. Never attempt to exit or jump from an out-of-control vehicle.
